Please read the form carefully, complete and return to the North Shore Specialist Day Surgery as soon as possible, at least 7 days prior to your admission. The completed form can be returned via mail (Level 1 176 Pacific Highway Greenwich, NSW 2065) or fax (02 9425 1655 – please be careful to fax both sides of the form).

Our patient admission personnel will telephone you the day prior to your procedure to confirm details for admission; explain your pre operative fasting requirements and discuss your general medical history and any medications.
